Installing a phone jack?

I would like to replace our old phone jack with a new one (a Levitron Quickport.) We have four wires we disconnected from the old jack. 2 blue and 2 blue/white. Which ones go on what spots on the Quickport? I do not see that there are corresponding colors on both sides of the color guides. One side has both blue and blue/white, but the other only has blue. Can someone please explain which colors I should line my preexisting lines up with? Thank you in advance!

Here's where the fun begins. The jack you took out was looped with both wires under a screw terminal, electrically, both wires were connected together.

The Leviton jack is an insulation displacement, which means you can only put one wire in the channel. The white blue goes into the white-blue or green slot, and the solid blue, or blue-white goes into the red slot. Your problem is that they need to be spliced together and you press the loop into the channel. ID jacks work really well, until you run into this application. We use inline splices (709-SD's) that splice a pair together, but twisting and tape will work in a pinch. Be sure to seat the looped wire firmly in the channel.
